NOTE: If you are a UT Health Science Center employee, these forms are already on record in the Human Resources Office. Notify us when you will resign your employment so we can make the transfer to student employee status.
For students traveling from a country outside the United States, these forms must be completed upon your arrival to the Health Science Center.
All other students: Please print and complete these forms. 2) United Student Health Insurance or call (800) 767-0700 for more information. Search for My Schools Plan on their website and enter UT Health Science Center to view insurance coverage plan.
3) UT Select Health Insurance students are part-time employees classified as teaching assistants and therefore, also have the option to purchase UTSelect (Blue Cross Blue Shield) medical insurance. Decisions regarding your selection of UTSelect must be made by August 31, 2008. Please contact the Human Resources Office at 210-567-2600 to speak to a benefits advisor or email Sandy Casanova at casanova@uthscsa.edu. A benefits presentation is scheduled for August 19, 2008.
